COVID 19 INFO
BE ADVISED:
-Temperature of 100 degrees or more
-Shortness of breath
-Loss of taste or smell
- Dry cough
- Sore Throat
2. Have you been in proximity or in contact with anyone with these symptoms or anyone COVID-19 POSITIVE in the past 14 days?
3. Do you live with anyone that is quarantined right now?
4. Have you traveled outside the United States in the past 14 days?
5. Have you traveled domestically within the US by airline, bus or train within the past 14 days?
6. Have you been directed to quarantine or isolate by the RI Dept of Health or healthcare provider in the past 14 days? If so, When does/did your quarantine or isolation period end?
*IF YOU HAVE ANSWERED YES TO ANY OF THESE QUESTIONS YOU WILL HAVE TO RESCHEDULE YOUR APPOINTMENT AFTER 14 DAYS HAVE PASSED OR YOU GET TESTED AND IT IS NEGATIVE.
*ALSO If you have had these symptoms in the last THREE DAYS - Cough, shortness of breath, fever above 100 degrees, chills, unexplained muscle pain, sore throat, headache, nausea or vomiting, diarrhea, runny or stuffy nose, extreme fatigue, loss of taste or smell - you will need to reschedule.
- Understand that Covid-19 virus has a long incubation period during which carriers may not show any symptoms and may still be HIGHLY contagious. It is impossible to determine if someone has the virus.
- Understand that due to clients and other people entering the building, the characteristics of this virus, and being in an enclosed room may elevate the risk of contracting the virus, simply by visiting the lash studio.
- You MUST answer NO to all of these questions in order to be seen by Cassie at Wicked Lash Design:

CLIENT RULES
1. Take your temperature before coming to the studio and let me know by text that it is normal.
2. Upon arriving at Wicked Lash Design, please remain in your vehicle or outside until you receive a text from me, telling you that I am ready for you.
3. You will be required to wear a mask for the entire duration of your appointment.
4. When you enter the office suite, please use the restroom and wash your hands thoroughly.
5. DO NOT go into the lash room until I have come out and taken your temperature with a no contact thermometer. If you have a temperature of 100.2 or more we will have to reschedule in no sooner than 14 days. If you get tested and are negative after this event, then you may reschedule sooner.
6. Once allowed to enter the room, please use the sanitizer provided to disinfect your hands.
7. Avoid touching anything you do not need to be touching. Please try to leave any extra articles such as handbags in the car or at home and come in with your payment method, keys and phone only if possible.
8. No using cell phones, vaping or other devices during your appointment.
9. CASH PAYMENT - if paying in cash, please have it all set in an envelope.
10. I have removed all the comfortable plush goodness of my table, so if you are prone to getting cold, you will need to bring your own blanket. Unfortunately this is how it needs to be for the time being, but please be assured I love making my clients comfortable and this is hurting me as much as it hurts you! :(
11. When the session is over and you have paid, please use the hand sanitizer before leaving the room and keep your mask on until you have left the building.
